All students who wish to try out for a sports team must have an up-to-date physical form in the athletic office at the school. Notes from healthcare providers will not be accepted.

Each child must have a sports physical within the year of the complete playing time of the sport. For example, if your child would like to try out for basketball in December 2012 then a physical exam is required anytime from April 1, 2012 until December 1, 2012, as the playing time is from December 2012 through March 2013.

Please check with your healthcare provider to see if your son or daughter had a recent physical. Students may have sports physicals done by their healthcare provider or walk-in clinic. Students must submit a copy of the sports physical to the athletic office prior to tryouts or practices.

Students are encouraged to have a sports physical completed during the summer to avoid any interruption in their ability to remain eligible for participation in the sports program.
